EDCHOICE APPLICATION PHASE 2

Please complete Phase 2 as soon as possible.  Phase 1 must be completed before beginning Phase 2.  

EDCHOICE APPLICATION PHASE 1

1. 2023-24 EdChoice Request Form

2. Proof of Address (Copy of a utility bill):

A current (less than 90 days old) utility bill. The utility (electric, gas, water, sewer, cable/internet) bill MUST SHOW MATCHING SERVICE AND MAILING ADDRESS in the name of the parent/guardian. Post office boxes (except in rural areas where residents only have a PO Box) and cell phone bill have no service address and therefore are not accepted.

3. Birth Certificate:

Provide 海角视频 with a copy of the student's birth certificate.

The EdChoice request form, proof of address, and birth certificate can be emailed to Rita Dominick at rdominick@sanduskycc.org , printed and turned into the secretary at the blue awning main entrance, or mailed to SCCS, 410 West Jefferson Street, Sandusky, OH 44870.
